I hereby grant irrevocably to 40 Day Management, Inc., and to any third parties it may authorize, and to its and their employees, agents, assigns and licensees, the absolute right to use and copyright my likeness, as my likeness is photographed and/or recorded on this date by any means whatsoever, for advertising, promotional or any other lawful purpose whatsoever, including but not limited to, the exhibition and/or broadcast, theatrically or non-theatrically or on any form of television including over-the- air, basic cable, pay TV, STV, video cassettes, video discs, satellite television, or radio, or any and all other media, or any motion picture film or video tape or audio tape in which my physical likeness may appear or be used or incorporated, and in connection with the publication in magazines, newspapers or otherwise, of any articles, promotions or interviews in which my physical likeness, may appear or may be printed, used or incorporated, without any further remuneration or liability to me. I hereby waive any right that I may have to inspect and/or approve the finished product in which my likeness, voice, name or biography appears, or the use to which it may be applied as these relate to the needs of the agency’s client.

I HEREBY RELEASE 40 Day Management, Inc., any third parties it may authorize and its and their employees, agents, assigns and licensees from all liability to me in regard to any form of  distortion  of my likeness, whether intentional or otherwise, which may be present in the finished product. I further agree to hold faultless 40 Day Management, Inc.., any third parties it may authorize and its and their employees, agents, assigns, and licensees for any personal injuries and/or damages that I may sustain while in the voluntary performance of the activities for which I am being compensated under this agreement.

This RELEASE shall be in effect as of the date of my signature below, and is in lieu of all other contracts or agreements, whether written or oral, expressed or implied.

I hereby certify and represent that I have read the foregoing and fully  understand the meaning and effect thereof and intend to be legally bound by this agreement
